Dans un premier temps, j'explique comment le changement de statut d'un hôpital psychiatrique étatique pour adultes, ayant un passé asilaire à connotation sociale d'abord – et plus précisément psychiatrique plus tard – en établissement public relevant du droit privé, a conduit à une crise institutionnelle dans laquelle des dysfonctionnements anciens ont été mis en mouvement par des initiatives nouvelles suscitant des mécanismes de défense primitifs et constituant l'origine d'une souffrance importante chez les membres de l'institution. Dans un deuxième temps, je décris la résilience au niveau d'une unité de soin. En conclusion, je discute les possibilités qu'une institution pourrait mettre en oeuvre pour pallier à la souffrance institutionnelle. Source : Éditeur (via Cairn.info) |

In this article, explanations are provided of the changes which occurred when a state psychiatric hospital, became a ``public institution with private rights". For many years, this hospital operated as an asylum and was suddenly required to undergo a process of modernization which emphasized initiatives for enhancing quality of care for the patients. The proposed changes brought about considerable anxiety and stress. It is postulated that these changes provoked an institutional crisis in which old, dysfunctional behavior became apparent, giving rise to primitive defence mechanisms. For the members of the institution, the perturbed relationship they have with the institution has brought about much pain and suffering. This article describes the ways in which one hospital ward was able to deal with the situation. In the conclusion, attempts are made to answer the question of what to do when an institution dysfunctions for a long time and when its inner resources do not suffice to find solutions to deal with the crisis.
