L'introduction du surmoi dans la seconde topique marque un nouveau tournant dans la pensée freudienne de la souffrance. D'abord présenté comme une différenciation à l'intérieur du moi issue de l'identification, le surmoi s'avère se retourner contre le moi et le soumettre à sa critique. Le dégagement de la pulsion de mort va transformer cette critique en cruauté, muant le surmoi en tortionnaire, dont la haine apparaît comme le résultat paradoxal de la quête d'amour du moi. The introduction of the super-ego in the second topic shows a new turning point in the freudian thought of suffering.
