Le secteur de la santé mentale connaît un certain nombre de difficultés et dysfonctionnements (Laforcade, 2016) qui paraissent souvent liés à des facteurs entremêlés d'ordre organisationnel, institutionnel et culturel. Diverses analyses scientifiques et opérationnelles, au plan international, incitent à explorer de nouveaux modèles de prise en charge, permettant une approche plus globale et mieux coordonnée, s'appuyant notamment sur l'imbrication des soins et des services sociaux et médico-sociaux. Cet article examine, à partir d'un cadre théorique interdisciplinaire (santé publique, psychiatrie, management et organisation) et d'une étude de cas approfondie, en quoi la mise en œuvre d'un dispositif structurel et managérial innovant et s'efforçant de répondre à des besoins identifiés, peut se heurter à des obstacles tels que des formes de conservatisme sociétal et diverses manifestations de résistance au changement. Ainsi, cette recherche exploratoire met en avant d'une part la faisabilité et l'opportunité de nouveaux modèles de prise en charge du patient en santé mentale, et également les défis à surmonter et les conditions de réussite de ce type d'évolution. The mental health sector is faced with difficulties and dysfunctions (Laforcade, 2016) which often appear to be related to combined factors of an organizational, institutional, and cultural nature. International scientific and operational studies encourage the search for new models of care, allowing more integrated and better coordinated approaches, and relying on the interweaving of care, social, and medical-social services. Using an interdisciplinary theoretical framework (public health, psychiatry, management and organization) and an in-depth case study, this article analyzes the implementation of an innovative organization that strives to respond to identified needs, and to overcome barriers such as reactions of societal conservatism, and various manifestations of resistance to change. This exploratory research highlights, on the one hand, the feasibility and the appropriateness of new models of care for the patient in mental health care, and on the other hand the challenges and success factors of this type of innovation.
