L'objet de cet article est de proposer des alternatives à l'organisation actuelle – et telle qu'elle est prévue dans l'avant-projet de loi « hôpital, santé, patients, territoires » – du système sanitaire français. Il approfondit et formalise des réflexions engagées dans le cadre d'un projet d'agence d'objectifs intitulé « quelles alternatives à la logique gestionnaire libérale à l'hôpital ? Tentatives de formalisation de l'information hospitalière », piloté sur plusieurs années par Jean Lojkine et Jean-Pierre Escaffre. Ce projet a donné lieu à plusieurs initiatives et publications, dont l'organisation à l'hôpital de Tenon, les 25 et 26 avril 2004, d'un forum syndical et scientifique, en collaboration avec la fédération CGT de la santé.Nous verrons dans une première partie la structuration globale proposée, avec pour élément central la création de Chambres territoriales de santé. La seconde partie sera plus particulièrement centrée sur la structuration hospitalière en communautés hospitalières de territoire et sur les liens de coordination et de coopération entre les différents acteurs du système, dans le cadre de réseaux de santé publique. An alternative organisation of the French health system? This article aims at offering alternatives to the current organisation of the French health system – as planned in the pilot study of the “hospital, santé, patients, territoires” law. It studies in depth and formalises reflexions set up by a research entitled “Quelles alternatives à la logique gestionnaire libérale à l'hôpital ? Tentatives de formalisation de l'information hospitalière », carried out by Jean Lojkine and Jean-Pierre Escaffre. This project has led to several initiatives and publications, among which the organisation at Tenon hospital of a forum, in collaboration with the CGT health union, gathering some scientists and unionists, on April 25 and 26, 2004.We will first study the general structuring put forward, revolving around the creation of Chambres territoriales de santé (“Territorial Health Chambers”). We will then focus on the structuring of hospitals within communautés hospitalières de territoire (“Territorial Hospital Communities”) and on the coordination and cooperation of the various actors involved, within public health networks.
