Cet article compare l'évolution de l'organisation politique et institutionnelle des systèmes d'assurance maladie français et allemand. Si, dans les années 1990, les deux systèmes ont plutôt divergé, une convergence est à l'œuvre dans les années 2000. Ce rapprochement est analysé en termes d'affirmation d'un gouvernement à distance de l'assurance maladie. Pour l'expliquer, l'accent est mis sur la structuration d'acteurs programmatiques liés à l'Etat et confrontés à des enjeux de pouvoir similaires dans les deux pays (limitation du rôle des caisses et des médecins pour rendre les systèmes d'assurance maladie plus efficients). Changes in the governance of health insurance systems: a Franco-German comparison This article compares the changes in the political and institutional organisation of the French and German health insurance systems. In the 1990s, the systems developed differently, but since then they have tended to converge. This rapprochement is analysed in terms of promoting a more centralised governance of health insurance. In order to explain this convergence, emphasis is put on the structuring of policy actors linked to the State and facing similar power issues in both countries (limitation of the role of the insurance funds and doctors in order to make the health insurance systems more efficient).
