Le psychanalyste travaillant en milieu hospitalier avec des patients souffrant d'affections médicales ne propose pas seulement des suivis psychothérapiques individuels ou des lieux de soutien et de réflexion pour les équipes soignantes, mais aussi tout un travail de « liaison psychodynamique » au sein du « setting à trois » qu'il forme avec le somaticien et le malade. Ce travail peut être rendu difficile par l'occultation des dimensions inconscientes et par la logique correctrice en médecine, par l'instrumentalisation du « psy », par les résistances du monde médical et la complicité d'une certaine psychologie à vocation strictement « humanitaire » ou « scientifique », et enfin par certaines considérations gestionnaires. When working in hospital with patients suffering from medical diseases, the psychoanalyst does not only offer any individual psychotherapeutic follow-up or any support for care team: he also proposes a “psychodynamic liaison” work through the particular setting he is forming with the physician and the patient. This work may be made difficult by the occultation of the unconscious dimensions and by the corrective logical in medicine, by the instrumentalization of the “psy”, by the defences of the medical world and the complicity of the “humanitarian” or “scientific” psychology, and by some managerial constraints.
