Question ancienne et pourtant toujours d'actualité, le diagnostic alimente débats et polémiques. La dernière version de l'échelle de classification du DSM n'a certainement pas apaisé ceux-ci. Dans le vaste champ de l'aide et des soins à l'égard de l'enfant et de l'adolescent, les questions liées au diagnostic transcendent souvent les aspects de prises en charge et de liens thérapeutiques ainsi que les multiples échanges entre professionnels concernés. Le diagnostic pédopsychiatrique est un processus dynamique, modifiable et momentané, les organisations pathologiques étant pour la plupart en transformation permanente. De manière spécifique et en fonction du processus développemental, la thématique du diagnostic en pédopsychiatrie renvoie plus à la question du risque évolutif qu'à la question d'un état pathologique en soi. S'appuyant sur une vignette clinique, l'article développe une réflexion sur ce que nous comprenons par diagnostic dans le domaine spécifique de la psychiatrie infanto-juvénile. Sont abordés les aspects généraux liés au diagnostic lui-même avant de souligner les places, fonctions, enjeux de l'enfant dans la société d'aujourd'hui. Puis, le thème même du diagnostic est discuté, en en montrant l'utilité et les risques. Former question and nevertheless always of current events, the diagnosis feeds debates. The last version of the scale of classification of the DSM did not certainly calm these. In the field of the help and care towards the child and towards the teenager, the related questions with the diagnosis often transcends the aspects of assumptions and therapeutic links as well as the multiple exchanges between professionals concerned. The diagnosis pedopsychiatric is a dynamic, modifiable and temporary process, the pathological organizations being for the majority in permanent transformation. In a specific way and according to the process développemental, the set of themes of the diagnosis in pédopsychiatrie returns more to the question of the evolutionary risk than to the question of a pathological state in oneself. Leaning on a clinical case, the article develops a reflection on what we understand by diagnosis in the specific field of the child-psychiatry. The general aspects connected to the very diagnosis are approached before underlining places, functions, stakes in the child in the today's society. Then, the theme of the diagnosis is discussed, by showing the utility and the risks.
